(2) $96.40 Part B Premium.

 

 

(3) Health Insurance Premiums including Medicare supplemental (Medigap), HMO, PPO and the Epic fees paid by applicant.

 

(4) Effective May 1, 2002 there is no longer an asset limit.

 

(5) Applicants can attest to their assets.  No documentation of resources is required.
